SPECIALTY SERVICES
INFUSION THERAPY
Many types of infusion therapy can be delivered and administered in the patient's home. Some examples include therapy for total parenteral nutrition, hydration, and IV antibiotic administration. To ensure safe and effective infusion therapy at home, Covenant's experienced nursing staff works together with the patient, physician and pharmacist to develop a plan of care to meet the patient's needs. The patient and caregiver receive education and support regarding the medication and administration to help them become as independent as possible.

WOUND CARE SERVICES
Covenant offers a specially trained, certified wound care nurse whose role is related to skin and wound care management of homecare patients.
Our certified wound care nurse provides:
- Skin and wound assessments
- Recommendation of treatment and supportive equipment to promote wound healing such as negative pressure wound therapy (VAC®)
- Education to patients and their caregivers on wound care management
Homecare patients who may benefit from the wound care program include those who have:
- Pressure ulcers
- Venous or arterial ulcers
- Diabetic ulcers
- Surgical wounds
- Drains
- Fistulas
- Incontinence/skin problems

MATERNAL/CHILD CARE
Covenant's maternal/child experienced nurses provide medical assessments, education and treatment for expectant and new mothers and their newborns. Teaching includes instruction on breastfeeding, formula preparation, and routine or specialized prenatal and postpartum care.

PEDIATRIC SERVICES
Covenant provides pediatric homecare services for children up to age 18. These services may include nursing care or therapy for acute and chronic medical conditions.
Examples of pediatric homecare services include:
- Post operative care
- Wounds/burn care
- Home infusion therapy
- Special care needs such as tube feedings, foley catheter care, apnea monitoring, ventilator or tracheostomy care
Covenant can provide special care services for children in their school or daycare.

MENTAL HEALTH SERVICES
Covenant's specially trained mental health nurses work with patients, their families and their psychiatrist/psychologist or physician to assess the patient's mental health needs and provide the necessary treatment for the patient to safely remain at home. Interventions focus on psychiatric and mental health promotion, administering and monitoring psycho biological treatment regimens, and psychological health teaching including crisis intervention, counseling and case management. Patients who may be appropriate for this program include those with:
- Depression
- Anxiety
- Illnesses requiring medication adjustments

HEART FAILURE PROGRAM
Covenant's Heart Failure Program includes patient and caregiver teaching and acute exacerbation treatment by a registered nurse in collaboration with the patient's physician. Our goal is to safely and cost-effectively manage heart failure patients in their homes, avoiding frequent emergency room visits and re-hospitalizations. Patients who may benefit from the program include:
- Those who are frequently admitted to a hospital (acute care facilities) due to aggravated or increased disease symptoms (exacerbations)
- Those who require extensive and ongoing teaching and monitoring of their disease
- Those requiring medication teaching/adjustments

TRANSITIONS PROGRAM
Covenant's Transitions Program is designed to meet the needs of patients facing potentially life threatening illnesses, who may or may not elect Hospice in the future. While under this home care program, patients are cared for by the nurses and social workers who are specially trained in areas common to these patients such as pain management, treatment concerns (such as chemotherapy and radiation issues for cancer patients), declining functional ability, psychosocial concerns, anticipatory grieving and financial concerns. The team has time to build a rapport with the patient to introduce the Hospice philosophy in a very gentle way.
Patients who may be appropriate for this program include:
- Those with a new diagnosis of cancer, especially if it has spread to other areas of the body
- Those with advanced or end-stage non-cancer diagnosis with repeated hospitalizations or emergency room visits and declining functional status
- Those who want to seek treatment beyond palliative care
