All aspects of patient centered care and services are coordinated to
promote
optimal planning and communication.
NURSING
Provided by a Registered Nurse or Licensed Practical Nurse with training and experience in providing care in the home.
- Teach, educate and train the patient and their caregivers
Examples include:
- Insulin preparation and administration
- Catheter care and irrigation
- Care of new bosomy
- Wound care, including negative pressure wound therapy (VAC®)
- Disease process management
- Health Promotion and disease prevention strategies
- Enteral feeding management
- One way valve chest tube management
- Home safety
- Intravenous therapy management
- Safe medication administration
- Observe, assess, and monitor health status and progress
Examples include:
- Initiation, change, or discontinuation of medication
- Changes in disease processes, such as heart failure
- Onset of a disease process such as diabetes or pneumonia
- Recent hospitalizations
- Wound assessment
- Mental Health
- Perform skilled interventions and treatments:
Examples include:
- B-12 injections
- Intravenous therapy administration
- Insulin injections
- Wound care, including negative pressure wound therapy (VAC®)
- Foley catheter care
- Manage and evaluate patient centered care
- Coordinate patient centered care & services with physicians & other health care team members

PHYSICAL THERAPY
Provided by a Licensed Therapist
- Provide patient centered care
- Teach, educate and train patient and caregivers regarding assistive devices
- Evaluate home safety
- Develop a home management/therapeutic exercise program
- Provide treatment for:
- restoration and strengthening
- gait, ambulation, balance and transfer techniques
- range of motion (active and passive)
- wound care and electrical stimulation
- ultrasound, massage and pain management
- prosthetic training

SPEECH LANGUAGE THERAPY
Provided by a Licensed Therapist
- Assess and provide treatment for cognitive (pertaining to mental processes) and congenital (meaning present at birth) disorders
- Develop retraining activities for patients with speech, voice and language production impairments (communication disorders)
- Develop retraining activities for patients with swallowing, breathing and muscle tone impairments (dysphagia disorders)
- Educate regarding prosthetics and alternate methods of communication

OCCUPATIONAL THERAPY
Provided by a Licensed Therapist or Licensed Therapy Assistant under the direction of the Therapist.
- Teach new ways or adaptations to increase and restore functioning, using exercises and activities
- Evaluate and educate regarding activities of daily living (ADL's) with the focus on patient centered care needs, such as:
- bathing
- grooming
- dressing
- feeding/light meal preparation
- light housekeeping/laundry

MEDICAL SOCIAL SERVICE
Provided by a Medical Social Worker
- Evaluate social and emotional factors affecting patients and caregivers
- Assess patient, family and caregivers ability to cope with current illness, including financial resources, social and emotional responses
- Identify available community resources and assist with coordination of these resources
- Provide short term counseling for the patient and caregivers

HOME HEALTH AIDE
Provided by a Certified Home Health Aide; experienced and trained in providing care in the home and delivered under the supervision of a Registered Nurse or Licensed Therapist
- Promote independence and/or assist with activities of daily living (ADL's) with the focus on patient centered personal care needs, such as:
- bathing
- grooming
- dressing
- feeding/light meal preparation
- light housekeeping/laundry
- assist with home exercise program

Provided by a Licensed Dietician
- Offer patient centered nutritional counseling and diet suggestions
- Assist patient and caregivers with specific dietary needs and food preferences
- Teach specialty diets

- Provide a variety of supportive services, such as:
- patient and caregiver support
