- MAKE A REFERRAL-

"If I could just get a little help, I would be alright to stay in my home."

"I am 85 years old. I have been living in the same home for over 60 years. Now I am faced with the possibility of having to leave this home because I am having a difficult time doing some things I always used to be able to do. It's personal, but I am having trouble bathing and fixing myself something to eat every day. I just get too tired.

My son had heard about Covenant Home Care and recommended that I call them. It was the best thing I could have done! I was pleased to learn that they would have a person, called a trained Home Health Aide, come out to my home and help me with the tasks I needed assistance with. My doctor is agreement with his plan too. Now I can continue my life as I have been while getting the help I need from Covenant Home Care.

I sure am happy I am able to stay in my home. Thanks Covenant Home Care. You made it possible!"

At Covenant Home Care we strive to provide a continuum of care to meet the individual needs of each person, at each stage in their life.

The Private Duty Program at Covenant Home Care supports the patient and their families when services are not covered by the medical insurance. Admission to the Private Duty program can only be made under the direction of a physician, based upon the care needs.

We involve the patient, caregiver or designee, key professionals and other staff members in developing an individualized plan of care. This plan is designed to increase one's ability to care for oneself in an effort to help the patient remain in their place of residence, if it is safe and reasonable to do so.

Our support may be provided in many ways, ranging from assessing patient progress and providing treatments, to giving medications. Individuals who can benefit from Private Duty services may include those who need:

  • medication management (fill medication planner)
  • Viniculture for lab work as ordered by your physician
  • personal care (may include bathing, shampooing hair, changing bed linen & assistance with other activities of daily living)
  • Vital signs / health status check

Private Duty services may be provided by a registered nurse, licensed practical nurse, therapist, social worker, health aides, companion or homemaker.

These services are provided on an hourly bases, twenty-four hours a day, seven days a week (based on staff availability). the fees are based on what services are provided and payment is an out-of-pocket expense for the patient / family. Private Duty expenses may qualify for Long Term Insurance coverage.

Please don't hesitate to contact us with any questions or concerns you may have. We are here to help you in any way we can.
