"If today is Saturday, why is dad taking Wednesday medication?"

"My dad is 84 years old and has suffered from chronic obstructive pulmonary disease (COPD) for more than 8 years now. Over the years, the doctors that care for him make changes to his medications to help control his condition. Lately, he has been having trouble keeping his medications organized. He is also sleeping more and not taking his shower regularly. I knew is was bad when he cancelled his trip with the senior club.

I talked to dad about seeing the doctor, but he said he was fine and just needed a little help. So I called Covenant Home Care. The agency contacted my dad's doctor to ensure the doctor was in agreement with the plans for home care services. The doctor was in agreement, knowing that my father would be evaluated by a qualified and certified home care agency.

As scheduled, a nurse from Covenant Home Care visited the home the next day and in coordination with myself, dad, and the doctor, developed a plan of care to meet my dad's health care needs. This plan included the recommendations for a physical therapist and a home health aide to help dad.

It has been several weeks now and dad is getting around a lot better. He is taking his medications correctly and no longer needs assistance to bathe. The therapist said by next week he would not need therapy services any longer. My father and I are very happy we made the decision for home health services. We felt assured that someone else, professionals, were helping with the management of his health care needs and to know what care Covenant Home Care was providing.

I know dad will be sad to see them go, but yesterday when dad was talking about the next senior bus trip, I knew he was feeling better."

Services provided are based upon the patient's identified care needs, home bound status and the type of services required that we can provide directly or through coordination with other organizations. Working with physicians, our qualified staff will plan, coordinate and provide care tailored to ones needs. Our services include: Skilled Nursing, Mental Health Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Services, Home Health Aide, Infusion Therapy and Nutrition Therapy.

We involve the patient, the caregivers or designee, key professionals and other staff members in developing an individualized plan of care, treatment and services. The plan is based upon identified problems, needs and goals, physician orders for medications, care, treatments and services, timeframes, environment and personal wishes whenever possible. The plan is designed to increase ones ability to care for oneself. Effective pain management is an important part of your treatment.

The plan may include the following interventions and goals, and is reviewed / updated as needed, based on ones changing needs:

• Personal Care
• Rehabilitation Therapy
• Psychosocial Needs
• Discharge Planning
