Who are we?
- A faith-based, non-profit community medical home care agency providing Home Care, Hospice and Private Duty services
- Formerly known as Holy Family Home Health and Hospice Care and St. Joseph Home Health
- Affiliated with the Catholic Diocese of Allentown and St. Joseph Medical Center in Reading
- Proud to have been providing faith-based skilled, compassionate care to our communities for over 30 years
What are we?
- Medicare-Medicaid certified, and licensed by PA Department of Health
- Services Provided:
HOME HEALTH: offers a wide range of services including Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Nutritionist, Medical Social Services and Home Health Aide. Home care is often recommended for people just returning from the hospital or those with new or chronic medical conditions requiring teaching and support. Covenant's services allow patients to stay in the comfort of their own home and receive high quality health care.
HOSPICE: offers a holistic, team approach of providing comfort care to patients with a terminal illness. Emphasis is on the quality of life for the patient and providing optimum support to the family and caregivers. The team consists of: Attending Physician, Hospice Medical Director, Registered Nurse, Medical Social Worker, Pharmacist, Home Health Aide, Volunteer, Chaplain and Nutritionist. Other team members, such as Physical, Occupational and Speech Therapy, are consulted on an as needed basis.
PRIVATE DUTY: offers services that may include Nursing (RN and LPN), Therapy, Home Health Aide, Homemaker and/or Companion. There is a fee for service, paid by the patient and/or long term insurance, for those who are not eligible for skilled services or who have custodial needs, such as requiring assistance with personal hygiene. Acceptance of patients is based on staff availability.

When are services available?
- 24 hours a day / 7 days a week
- Regular office hours are Monday - Friday, 8:00 am - 4:30 pm
- After hours on-call Registered Nurses
